Traditional Funeral Service
ATraditional Service offered by Laurel Oaks Funeral Home consists of:
services of funeral director and staff, embalming, other preparation of
remains, use of stateroom for viewing, use of staff, facilities and/or
equipment for the funeral ceremony in our chapel or church of your choice,
transfer of remains to our funeral home, funeral coach, utility vehicle, clergy
car, memorial register book and acknowledgement cards.
Note:Traditional Funeral Service is within a 50 mile radius of Laurel Oaks Funeral
Home. Additional mileage will be $2.50 per mile, one way, per vehicle.
The total charge for these services is $3,695.00

| A Traditional Service offered by Laurel Oaks Funeral Home consists of services of
funeral director and staff, embalming, other preparation of remains, use of stateroom for
viewing, use of staff, facilities and/or equipment for the funeral ceremony in our chapel
or church of your choice, transfer of remains to our funeral home, funeral coach, utility
vehicle, clergy car, memorial register book, and acknowledgment cards.

Home. Additional mileage will be $2.50 per mile, one way, per vehicle.
The total charge for these services is $3,695.00
The goods and services shown below are those we can provide to our customers. You
may choose only the items you desire. However, any funeral arrangements you select
will include a charge for our basic services and overhead. If legal or other requirements
mean you must buy any item you did not specifically ask for, we will explain the reason
in writing on the statement we provide describing the funeral goods and services you
selected.
You may choose only the items you desire. If you are charged for items you did not
specifically request, we will explain the reason for the charges on the written
memorandum. Please note that there may be charges for items such as cemetery fees,
flowers, and newspaper notices.
A. Charge for Services Selected
1. Professional Services

| Basic Services of Funeral Director and Staff |

Our fee for the services of funeral director and staff includes, but is not limited to: staff to respond to
initial request for service, arrangement conference with family or responsible party, preparation and
filing of necessary authorizations and permits, recording vital statistics, preparation and placement of
obituary notices, staff assistance prior to, including coordination with those providing other portions of
the funeral, (i.e. cemetery, crematory, and others). Also included in this charge are overhead expenses,
relative to our facility such as insurance, maintenance and utility expenses, secretarial and
administrative costs, and equipment and inventory expenses.
This fee for our basic services and overhead will be added to the total cost of the funeral arrangements
you select. (This fee is already included in our charges for Direct Cremation, Immediate Burial, and
Forwarding or Receiving Remains.).

| Embalming |

| Embalming is not required by law. Embalming may be necessary, however, if
you select certain funeral arrangements, such as a funeral with viewing. If you do not
want embalming, you usually have the right to choose an arrangement that does not
require it, such as direct cremation or immediate burial. |

Cosmetology, restoration when necessary, dressing and casketing of the body,
hairdresser. |

It is the policy of our firm to require refrigeration of an

Laurel Oaks’ policy is that human remains will not be held at our facility pending disposition arrangements
beyond a 24 hour period from its arrival without being embalmed or placed in refrigeration. In the event you are
not comfortable with our policy as outlined, you have the right to make arrangements with another funeral home.

2. Facilities and Equipment

Visitation
Use of facilities and staff services for visitation. Our services include: set-up of
visitation area, placement of casketed remains, display of floral arrangements,
and supervision of attendance during the visitation. (Maximum 2 Days)

Use of chapel and staff for funeral services (with or without body present) or
additional staff necessary for services in other facility. Our services include
coordinating the funeral service, supervision of funeral, and staff to attend

Memorial Service
Our charge for memorial service, if Laurel Oaks did not provide final
disposition of the deceased, (without body present) in either our funeral
home chapel or church includes the professional services of our staff, personal
supervision of all memorial arrangements, arrangement of floral tributes,
consultation with the family and clergy, and other staff services as may be
required. |

Graveside Service
Equipment and staff services for graveside service. Our services include
accompaniment of remains to cemetery, supervision of graveside service,
and staff to attend service.

Forwarding of Remains

This charge includes transfer of remains to our funeral home from a point within a
50 mile radius, services of funeral director and staff, embalming, administrative
services, and local transportation to airport (excluding shipping charges). It does
not include a receptacle for the remains, the services of staff of the use of facilities
for any visitation and/or rites or ceremonies prior to forwarding of the remains.

Direct Cremation
Direct cremation includes transfer of remains to our funeral home from a point within a 50 mile
radius, securing doctor's signature, filing death certificate with proper authorities, securing
cremation authorizations and/or permits, services of funeral director and staff, crematory charges,
alternative container, refrigeration (if necessary) and medical examiner permits. It does not
include facilities, rites, or ceremonies. * A $100.00 paperwork fee will be charged for deaths
occurring outside of Dallas County.
If you want to arrange a direct cremation, you can use an alternative container. Alternative
containers encase the remains and can be made of materials such as fiberboard of composition
materials (with or without outside covering). The containers we provide are fiberboard.

Immediate Burial
The charge for immediate burial includes transfer of remains to our funeral home from a point
within a 50 mile radius, local transportation to cemetery, necessary authorizations, and services of
funeral director and staff. Immediate burial means to go to the cemetery as soon as possible after
death has occurred. Viewing ceremonies, rites, services, procession to the cemetery, refrigeration,
etc., are not a part of the immediate burial and you are not being charged for those services. The
time of burial will be at a time convenient to the funeral home. NOTE: Refrigeration will be
required after 24 hours. |
